Adjudication & Equipment Form

This web page will allow for a group director to submit their SPRING IN THE PARK MUSIC FESTIVAL Adjudication and Equipment Request Form information online. 

This webpage asks for the same information as the paper copy sent to each director within their Final Packet of information.

Before you begin- we must know to whom this information belongs.  What school group is submitting this form?

PERFORMING GROUP INFORMATION:

Each director was sent a separate Adjudication and Equipment Form for each performing group that is listed on your Invoice and Group Information Form.  

Please complete this form for EACH PERFORMING GROUP that will be performing at the music festival.  After providing all the applicable information for your performing group- please click the SUBMIT button at the bottom of this webpage.  A confirmation page will show the information sent to the festival office.

Completing this form for multiple groups?  After you have clicked the SUBMIT button for a performing group- a confirmation page will appear showing the information sent to the festival office.  At the bottom of this confirmation page- a "return to form" link can be selected to begin providing information for the next performing group.  Complete this form again with the next group's information.  Then click SUBMIT to again send.  Repeat as necessary.    

  Performance Type and Group Number as listed on your Invoice and Group Information Form ("1st" should represent your top group in that Performance Type while "2nd" should indicate the second or Non-Varsity group)
