Please out this form if you are requesting info about the upcoming Winter Park Ski-Music Festival and would like to receive more information.

Trip Info Form

Please fill out this form ONLY AFTER you have mailed in your entry form and entry fee/housing deposit AND when requested to do so by the Festival Office.

Group Info Form

Please fill out this form ONLY AFTER you have received your binder packet and have been instructed to do so by the Festival Office for JANUARY payment and info.

Adjudication Form & Equipment Form

Please fill out an adjudication form for EACH performing ensemble/group.

The Final Group Info Form cannot be completed online.  It must be mailed with your rooming list, insurance list and money.
